So, today we’re talking about risk-based quality management, or RBQM. Now whatis it and what problem is it trying to solve for clinical data management teams?

Jullia
So Risk-Based Quality Management, or RBQM as you mentioned, is a structured wayto manage quality by focusing attention where it matters most. In clinical datamanagement, that means you identify the data and processes that are critical toparticipant safety and to reliable trial results, then you actively look forthe risks that could compromise them. The practical outcome is earlierdetection through defined signals and thresholds, rather than finding issueslate through broad, manual clean-up.

Now you mentioned critical data and critical processes. How do teams decidewhat counts as ‘critical’ in practice, especially when protocols generate hugevolumes of data?

Jullia
Well the starting point is the study objective and what you need to trust toanswer it, alongside what could affect participant safety. Critical data oftenincludes primary endpoint data, key safety data like adverse events, and anydata that drives key decisions during conduct. Critical processes, on the otherhand, are the workflows that protect that data, such as how it’s collected,entered, reviewed, corrected, and stored. Once you’ve named those, you ask asimple question. What could go wrong that would have a meaningful impact? Indata management, that might be delayed entry that causes missing context,unusually high query rates that signal misunderstanding at site, or frequentlate corrections that could raise audit trail questions. RBQM gives you adisciplined way to decide which risks are worth active monitoring, and whichcan be managed with routine controls.

Now how does this connect to what regulators expect? People sometimes thinkRBQM is mainly a monitoring topic, but data management is right in the middleof it.

Jullia
You’re right, and expectations have moved steadily towards proportionate,risk-based oversight. Current good clinical practice guidance emphasisesquality management that focuses on factors critical to quality, and risk-basedapproaches that are fit for purpose. And that doesn’t mean “do less”. It meansdesign your systems and processes so they match the risk to participants andthe importance of the data you’re collecting. From a data perspective, thatbrings two things into focus. First, documentation that shows you’ve identifiedand assessed risks and put controls in place. Second, evidence that you’rereviewing signals during the trial and acting when thresholds are breached.It’s about demonstrating control, traceability, and a rationale for why you’refocusing effort where you are.

Now when teams talk about RBQM tools, they usually mention KRIs, QTLs, andcentral monitoring. In data management terms, what do those look like day today?

So, Key Risk Indicators, or KRIs, are measurable signals that track potentialrisk exposure. They’re meant to trigger action. Common examples include dataentry timeliness, query rates, protocol deviations, and missing data incritical fields. You might also track adverse event reporting timeliness andcompleteness, site drop-out rates, or patterns of late data correctionsreflected in the audit trail.
Quality Tolerance Limits, or QTLs, are slightly different. They tend to behigher-level thresholds that define what level of deviation is acceptable forfactors considered critical to quality. When you combine those with centralreview techniques, you can spot unusual patterns across sites or regions. Thenyou decide whether you need targeted training, a process change, or focusedfollow up.

Now how do you stop KRIs becoming noise? A lot of teams start with goodintentions, then end up with dozens of metrics and constant alerts.

So that’s one of the most common failure modes. A useful KRI is specific,measurable, actionable, relevant, and timely. If a metric can’t drive adecision, it probably shouldn’t be a KRI. Now a good way to pressure test it isto define, in plain language, what action you’ll take if the threshold isbreached. For example, if data entry timeliness slips beyond the threshold at asite, the action might be a targeted site communication, a refresher onworkflow expectations, or operational support to remove bottlenecks. If queryrates spike, you might review whether the issue is the eCRF design, protocolcomplexity, training gaps, or inconsistent source documentation. It also helpsto keep KRIs linked to the risks you documented. If the risk isn’t meaningful,the metric won’t be either.

Now you’ve mentioned documentation a couple of times. What should the ‘plan’look like for RBQM in CDM, and how do teams keep it from becoming static?

Jullia
So most teams will capture this in a risk monitoring plan that records therisks, how you assessed them, and how you’ll monitor and control them. The keypoint is that it’s used throughout delivery rather than filed away afterstart-up. It should be revisited as the trial evolves, as new data comes in, oras circumstances change. For example, shifts in operational delivery,technology updates or changes in expectations. From a CDM perspective, you’llwant clarity on what you’re monitoring, how often you review it, whatthresholds apply, who reviews it, and what happens when something breaches. Thenyou also want a record of decisions. If you change a threshold or replace ametric, you document why. That way, the plan stays practical and traceable.

Let’s talk about risk control. Once you’ve identified and prioritised risks,what are the main controls teams use in data management?

Now there are a few core controls. One is technology and automation. Usingelectronic data capture systems with real-time validation, automated editchecks, and reporting that supports ongoing oversight. Another is integration,where data from EDC or clinical trial management systems can feed into riskmonitoring tools so you’re not relying on manual compilation. Then there’sprocess standardisation. Consistent workflows across sites and teams reducevariation, which is where a lot of preventable quality issues start. Targetedtraining is also important, so people understand what the indicators mean andwhat to do when thresholds are breached. Finally, you need regular reviews,audits, and interim checks that look for deviations from expected patterns. Thepoint is to detect issues early enough that corrective action protects thetrial, rather than creating avoidable rework late in delivery.

Now how does RBQM change the relationship between data management, clinicaloperations and other functions? Because many of these signals sit across teams.

Well it forces clearer alignment, in a good way. Many risks don’t belong to onefunction. Take missing data for example. CDM might see it first, but resolutionoften depends on sites, monitors, or operational decisions. And considerprotocol deviations. They may show up in data patterns, but the root causecould be training, visit scheduling, or complexity in the protocol design. RBQMworks best when roles are clear, communication is consistent, and actions areagreed in advance. If a KRI breaches, the question shouldn’t be “who ownsthis?” It should be “what’s the response, and who’s accountable for each step?”That shared visibility reduces rework and helps teams focus effort onprevention rather than reaction.

We’ve talked a lot about monitoring and response. How do you keep the systemevolving over time, especially across longer studies where patterns change?

Jullia
You’d build in review and refinement as part of the operating model. KRIs needongoing calibration. Early in a study, data entry timeliness might be your mainconcern as sites ramp up. Later, the focus might shift to consistency, latecorrections, or specific safety reporting patterns. So, you’d review whetherthe indicators are still relevant, whether thresholds need adjustment, andwhether the response actions are effective. If the same issue keeps happening,that’s a sign you need a deeper corrective and preventive approach, not justrepeated query cycles. You’d also keep an eye on systems and processes beingfit for purpose. If you introduce a new vendor feed, add decentralisedelements, or change data flow, your risk picture changes. The point of RBQM isto adapt with the study.

Of course. Firstly, RBQM in clinical data management is about focus. You shouldbe explicit about what’s critical to participant safety and to the reliabilityof your results, then prioritise the risks that could meaningfully compromisethose areas. If everything is ‘critical’, you end up diluting oversight andmissing what matters. Second, keep monitoring actionable. Use a small set ofKRIs and higher-level thresholds where appropriate and agree responses inadvance so thresholds actually lead to decisions and not just reporting. Third,keep the risk monitoring plan current. Build a review cadence, capturedecisions and rationale, and adjust as the study evolves. Especially when dataflows, vendors, or operating models change.
And in terms of pitfalls, the big ones are overloading teams with metrics,treating the risk assessment as a start-up exercise, and relying on late-stageclean-up instead of preventing recurring issues.

If your indicators lead to timely action, your processes areconsistent, and your decisions are traceable, you’re much closer to the intentof risk-based quality management than if you’re just reacting late.
